General Information on the Relaunch of the Deafness Notification Database
This document gives you general information on the reintroduction of the Deafness Database and contains information for audiologists on how to submit information.
Deafness Notification Consent Form
Here Audiologists can download a consent form, which they need to get signed by the child or young person's parents/guardians. Audiologists need to keep this consent form on file.
Notifying cases to the database
Audiologists can enter notification information directly into the database which can be found online
here.
Alternatively audiologists can print a PDF of the notification form
from here
and complete this on paper with the family before entering the data into the online form.
